WHERE'S MY FILE?
If you were a previous patient of Dr. Rawana and Associates at 50 Bloor St. West, then your file has been ported to the Kodak Lens Vision Centre at 364 Yonge St., no matter which doctor you had seen at 50 Bloor St. West before.
You should have received a communication about the physical change in location of your file as of June 1st, 2017, from either Dr. Rawana himself, or from the new custodian optometrist, Dr. Rekha Misra, at 364 Yonge St.
PUTTING YOUR FILE WHERE YOU WANT IT
The information in your file belongs to you, and it is your right to request copies for yourself or for transfer to a different practice should you choose to do so! There may be an administration fee involved from the custodian practice (as it usually does involve a considerable amount of work to transfer each file).
